MD General Medicine
Direct answer
MD General Medicine is India's principal postgraduate qualification in adult internal medicine — the diagnosis and non-surgical management of diseases of the heart, lungs, kidneys, liver, endocrine system, blood and infections. It is a three-year degree pursued after MBBS and the compulsory rotating internship, with seats allotted through counselling on NEET-PG ranks or INI-CET for the AIIMS group. The degree qualifies a doctor to practise independently as a physician and to proceed towards DM super-specialisation.
Course structure and curriculum
- Ward residencies managing adult medical inpatients from admission to discharge.
- Emergency postings covering myocardial infarction, stroke, diabetic emergencies and poisoning.
- Critical care rotations in intensive and high-dependency units, including ventilation and shock management.
- Cardiology training: electrocardiography, bedside echocardiography basics and heart-failure clinics.
- Neurology exposure: stroke care, epilepsy, central nervous system infections and lumbar puncture.
- Nephrology units: acute kidney injury, chronic kidney disease, dialysis and electrolyte disorders.
- Endocrinology and diabetes clinics, insulin regimens, thyroid and adrenal disease.
- Respiratory experience including pleural aspiration, tuberculosis and obstructive airway disease.
- Gastroenterology and hepatology postings with endoscopy lists and liver clinics.
- Haematology and oncology day care with chemotherapy protocols.
- Infectious disease, HIV and fever clinics with antibiotic stewardship.
- Academic sessions: journal clubs, seminars, clinico-pathological conferences and grand rounds.
Entrance examinations
Almost all MD General Medicine seats are filled through NEET-PG, the computer-based test conducted by the National Board of Examinations in Medical Sciences (NBEMS). The Medical Counselling Committee (MCC) allots the All India Quota and central or deemed university seats, while state authorities fill state-quota seats; the AIIMS campuses, PGIMER Chandigarh and JIPMER Puducherry admit through INI-CET instead. General Medicine is among the most competitive branches, so aspirants need near-complete command of final-year clinical subjects plus strong para-clinical revision.
University examinations
Assessment runs on two tracks: periodic internal tests, case presentations, journal club evaluations and logbook sign-offs through the three years, and a final university examination. A research dissertation is compulsory under NMC norms — protocol framed early, data collected over the residency, accepted thesis submitted before the finals. The exit examination has theory papers on medicine and its sub-specialities, then practicals with a long case, short cases and viva voce spanning instruments, drugs, imaging and laboratory reports. Preparation must combine textbook depth with rehearsed case presentation.
Career pathways after MD General Medicine
Most graduates become consultant physicians in hospitals or independent practice, many taking senior-resident posts first to deepen critical-care exposure. The degree is also the gateway to DM and DrNB courses — cardiology, neurology, gastroenterology, nephrology, endocrinology, medical oncology, critical care — entered through NEET-SS or INI-SS. Corporate hospitals, railway, defence and ESI services and public health programmes are further employers.
