MCh Surgical Oncology
Direct answer
MCh Surgical Oncology prepares surgeons to resect and manage cancers of the breast, gastrointestinal tract, thorax, head and neck, soft tissues and other sites, always inside a multidisciplinary oncology framework. Like its sister super-specialities, it spans three years after MS General Surgery, subject to the feeder list attached to the year's NEET-SS groups. NBEMS's NEET-SS examination is the standard admission route, with INI-SS covering cancer-centre seats at AIIMS and comparable institutions. Training centres on achieving clear margins, staging accurately and coordinating with medical and radiation oncologists.
Course structure and curriculum
- Principles of surgical oncology: tumour biology, staging systems, margin philosophy and multimodality sequencing.
- Breast cancer surgery: modified radical mastectomy, conservation surgery and oncoplastic techniques.
- Gastrointestinal cancer resections: gastrectomy, colectomy and rectal surgery with total mesorectal excision.
- Oesophageal and thoracic procedures, including transhiatal and transthoracic oesophagectomy.
- Head and neck oncology resections with neck dissection.
- Soft tissue sarcoma management and limb-salvage philosophy.
- Skin cancers, melanoma and rare cutaneous tumours.
- Urological and gynaecological malignancy exposure through rotations or joint operating lists.
- Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy where available.
- Reconstructive collaboration: flap selection and coordination with plastic surgical colleagues.
- Palliative surgical skills: stoma creation, bypass procedures and symptom-directed operations.
- Protocol-driven thesis research concluded before the final assessment.
Entrance examinations
Selection rests on NEET-SS, the computer-based super-speciality entrance conducted by NBEMS. A candidate works through 150 multiple-choice questions in two hours and thirty minutes, gaining four marks per correct option and losing one for every mistake. The syllabus is that of the candidate's broad speciality at exit level, which for this course ordinarily means general surgery. Oncological surgery courses occupy a defined group under NEET-SS; all candidates whose qualifying degrees fall in that group contest the same seat pool through the Medical Counselling Committee's centralised rounds. INI-SS, administered by AIIMS New Delhi, fills surgical oncology positions at the Institutes of National Importance. Both tests punish guesswork, so disciplined omission of doubtful items is itself a scoring skill.
Examinations during and after the course
Progress is documented with operative logs, case discussions, tumour-board presentations and departmental assessments at intervals. The university finale pairs theory papers with clinical examinations, operative case discussions and vivas; thesis clearance is a precondition. Last-mile readiness usually means memorising staging tables, rehearsing operative steps and complications, and being able to defend adjuvant treatment decisions for every logged case.
Career pathways after MCh Surgical Oncology
Trained surgical oncologists are recruited by comprehensive cancer centres, oncology wings of corporate hospital chains and government cancer institutes. Fellowships in breast oncology, hepatopancreatobiliary cancer, peritoneal surface malignancy or thoracic oncology deepen expertise. Teaching posts, clinical trial leadership and cancer-programme directorship represent the mid-career ladder, while some graduates choose focused single-disease practices.
