Occupational Therapy in Mental Health
On this page
Direct answer
Occupational imbalance, deprivation and alienation — not hallucinations themselves — are the targets of occupational therapy in psychiatry: the symptoms medicine treats, and the broken daily routines those symptoms leave behind. Assessment builds an occupational profile through MOHO-based tools such as the MOHOST, the Occupational Self Assessment and the Interest Checklist, plus the Allen Cognitive Level Screen, and the wider team's scales (PANSS, BPRS, Hamilton depression and anxiety ratings) which the therapist must read even though others administer them. Intervention then rebuilds volition, habituation and performance through activity scheduling, graded daily-living groups, social and life-skills training, token economies on wards, sensory modulation and vocational linkage. India's rights-based Mental Healthcare Act 2017 and the District Mental Health Programme frame where this work happens.
What you must remember
- Lineage: moral treatment under Pinel and Tuke's York Retreat (1796), the arts-and-crafts movement, and Eleanor Clarke Slagle's habit training — the psychiatric roots of the whole profession.
- Allen Cognitive Levels (1-6): screened at the bedside with leather-lacing stitches — running, whipstitch, single cordovan, double cordovan — each stitch demanding a higher cognitive mode; a quotable OT-specific scale.
- MOHO assessments: MOHOST for observation across settings, Occupational Self Assessment for the client's own view, Interest Checklist, Volitional Questionnaire, and the Assessment of Communication and Interaction Skills for group behaviour.
- Activity scheduling for depression: plan pleasure and mastery activities into the week in advance — the behavioural engine of OT for low mood.
- Token economy: Ayllon and Azrin's behavioural system of earned reinforcers for target behaviours, used on structured wards.
- Sensory modulation: sensory rooms and weighted or deep-pressure strategies for arousal regulation in acute and forensic settings.
- Mental Healthcare Act 2017: rights-based care, advance directives, supported decision-making, and the decriminalisation of attempted suicide; National Mental Health Programme dates to 1982 and the District Mental Health Programme carries services to district level.
Rebuilding a day: schizophrenia at eight weeks
A 32-year-old man with schizophrenia, two relapses behind him, sits through the interview with folded hands and a blank calendar. Medication has quietened the voices; what remains is the wreckage of routine — waking past noon, meals skipped, the tailoring shop long since surrendered. The MOHOST confirms the pattern: interests present but dormant, roles collapsed, habituation the broken layer. The plan works outward from one anchor. First, a fixed wake time anchored to a valued outcome — chai with his father — because volition, not discipline, sustains habit. Second, a weekly planner populated with three graded occupations: a morning cooking group on the ward (sequencing, tolerating others, a real product), a forty-minute evening walk with a peer, and Saturday reconnection with a cousin who still stitches. Third, medication management woven into the same planner, with a pill organiser and a phone alarm, because adherence is an occupation too. The therapist reviews the COPM ratings with him at four weeks — his own scores, not the team's impression — and the tailoring question is revisited only once dressing, cooking and travel are stable, via the district vocational centre. Recovery here is measured in restored roles, not reduced psychosis.
Where students slip
Two slips recur in orals. The first is boundary confusion: asked how OT differs from clinical psychology, candidates talk modalities instead of targets — psychotherapy restructures thinking through talking, while occupational therapy restructures daily life through doing, and both may run in parallel for the same person. The second is the "keeping busy" fallacy: any activity that is not analysed, graded against a named deficit and tied to a documented goal is diversion, not therapy, and examiners will strip it bare in the viva. A safe habit is to never name an activity without naming what it is treating.
Frequently asked questions
Which activity does the Allen Cognitive Level Screen use?
Leather-lacing stitches — running, whipstitch, single cordovan and double cordovan — whose difficulty places the person across Allen's six cognitive levels.
What is occupational deprivation?
Prolonged exclusion from meaningful occupation due to external constraint — institutionalisation, stigma, poverty — rather than personal choice.
Which Indian law governs mental health rights?
The Mental Healthcare Act 2017, which is rights-based, permits advance directives and supported decision-making, and decriminalises attempted suicide.
Who developed the token economy?
Ayllon and Azrin, as an applied behavioural system of reinforcers for target behaviours on psychiatric wards.
Name three MOHO-based assessments used in psychiatric occupational therapy.
The MOHOST, the Occupational Self Assessment and the Interest Checklist (also acceptable: Volitional Questionnaire, Assessment of Communication and Interaction Skills).
