The eye begins on day 22 as optic grooves of the forebrain that become optic vesicles; each invaginates into a double-layered optic cup while the overlying surface ectoderm forms the lens placode and vesicle. The retina and optic nerve are brain derivatives, the lens and corneal epithelium surface ectoderm, and the choroid, sclera and vitreous mesenchyme. The ear develops in three parts: the membranous labyrinth from the otic placode, the middle ear and tube from the first pharyngeal pouch, and the external meatus from the first cleft with the auricle from six hillocks of the first two arches.

  • Optic cup: the outer layer becomes pigment epithelium of the retina, the inner layer the neural retina; the rim forms the ciliary body and iris epithelium.
  • Choroid fissure: the ventral gap carrying the hyaloid artery closes by about week seven; persistence leaves a coloboma of iris or choroid, and the artery regresses to the hyaloid canal.
  • Lens: the lens vesicle forms by week five; posterior cells elongate into primary fibres, anterior cells remain epithelium, and the lens grows throughout life.
  • Coats: ciliary body, choroid and sclera condense from mesenchyme; corneal epithelium is surface ectoderm, the rest of the cornea and the anterior chamber mesenchymal.
  • Optic nerve: ganglion cell axons grow back along the optic stalk; as a central nervous system tract myelinated by oligodendrocytes, it suffers demyelination in optic neuritis.
  • Internal ear: the otic placode invaginates and detaches as the otic vesicle in week four; its utricular part forms the semicircular ducts and utricle, the saccular part the cochlear duct and saccule, the organ of Corti differentiating by weeks 20 to 24.
  • Middle and external ear: the first pouch forms the tympanic cavity, mastoid antrum and auditory tube; malleus and incus from the first arch, stapes from the second; the first cleft forms the external acoustic meatus, and six auricular hillocks, three on each arch, fuse into the auricle.

Common confusion

The eye's dual origin is the perpetual trap: retina and pigment epithelium are neuroectoderm, lens and corneal epithelium surface ectoderm, everything else mesenchyme — retinal detachment occurs between the two neuroectodermal layers. In the ear, the sensory patches are placodal, the ossicles arch cartilage and the tympanic cavity pouch lining; low-set or malformed ears signal first and second arch disorders.
