Direct answer

Two morphological forms define this family of obligate intracellular bacteria: the small, dense, extracellular elementary body that infects, and the larger, metabolically active reticulate body that divides within a host-cell inclusion. Chlamydia trachomatis serovars A-C cause trachoma, the world's leading infectious blindness, which India's national surveys have now pushed to elimination levels; serovars D-K cause genital infection, neonatal conjunctivitis and pelvic inflammatory disease; serovars L1-L3 cause lymphogranuloma venereum. Because the cell wall lacks muramic acid and the organism pirates host ATP, beta-lactams are useless and tetracyclines or macrolides are the treatment; nucleic acid amplification is the diagnostic standard.

Why the life cycle dictates every fact

Every examination-worthy property of chlamydiae is a consequence of their cycle. The elementary body is metabolically inert and built for survival outside cells — that is the form transmitted sexually, perinatally and by flies in trachoma. Once inside an endocytic vacuole it converts to the reticulate body, which cannot make its own ATP and draws on the host — hence "energy parasites", hence also the target of therapy: tetracyclines and macrolides act on the protein synthesis that fuels reticulate-body division, while beta-lactams find no peptidoglycan to attack. The 48-72 hour cycle explains why cultures must be held, why inclusions grow visibly before they burst, and why the glycogen-packed inclusions of C. trachomatis alone take up iodine.

The cycle also organises diagnosis. Culture needs living monolayers because chlamydiae replicate nowhere else — technically demanding, which is why NAAT, unfussy about viability, has displaced it for genital infection screening. Serology with microimmunofluorescence still distinguishes species-specific antibody when NAAT is unavailable. At the bedside, apply the serovar map: a newborn with conjunctivitis in week two or three of life has serovars D-K acquired at delivery, while a villager with pannus and corneal scarring carries serovars A-C from childhood; a man with matted inguinal buboes and a draining fistula completes the picture with L1-L3. One species, three clinical universes, all navigated by the same intracellular cycle.
