Estimating GFR and Assessing Kidney Function
On this page
Direct answer
Estimating GFR means calculating glomerular filtration rate from serum creatinine or cystatin C, most often with the CKD-EPI equation, because direct measurement is impractical for routine care. Creatinine depends on muscle mass, diet and tubular secretion, so the estimate can mislead in special populations. Cockcroft-Gault gives a creatinine clearance used mainly for drug dosing, while measured clearance remains the reference standard.
What you must remember
- CKD-EPI: the standard eGFR equation for reporting and staging, using creatinine, age and sex; the 2021 version dropped the race coefficient.
- Cockcroft-Gault: creatinine clearance equals (140 minus age) multiplied by weight, divided by 72 multiplied by serum creatinine, and multiplied by 0.85 in women.
- Cystatin C: produced by all nucleated cells and less dependent on muscle mass, but raised by steroids, smoking, thyroid dysfunction and inflammation.
- KDIGO stages: G1 at 90 or above, G2 60 to 89, G3a 45 to 59, G3b 30 to 44, G4 15 to 29 and G5 below 15 mL/min per 1.73 m2.
- Creatinine traps: low muscle mass, high muscle mass, vegetarian diet, creatine supplements, and trimethoprim or cimetidine, which block tubular creatinine secretion.
- Non-steady state: equations are invalid in acute kidney injury, because eGFR overstates true function while creatinine is rising.
- Measured GFR: clearance of an exogenous marker such as iohexol, used when estimates conflict with the clinical picture.
A stepwise approach to choosing the estimate
The right method depends on whether kidney function is stable and what decision is being made. The steps below move from routine staging to the uncommon situations that need measured clearance.
-
Confirm a steady state. Equations are valid only when creatinine is stable. During acute kidney injury, a rising creatinine means the eGFR overstates the true filtration rate, so repeat measurements are needed.
-
Calculate eGFR with CKD-EPI. Report the value indexed to 1.73 m2 and stage CKD only when abnormalities persist for more than three months, since a single low value may be transient.
-
Use Cockcroft-Gault for drugs. The creatinine clearance guides dosing of renally cleared drugs, and the choice of weight, actual or adjusted, must be made deliberately in obesity.
-
Switch to cystatin C when creatinine misleads. Use a cystatin C or combined creatinine-cystatin C equation in low or high muscle mass, amputation, cachexia or when drugs distort creatinine.
-
Measure GFR when estimates conflict. Reserve measured clearance using an exogenous marker for diagnostic conflict, transplant donor assessment and research, since it is more accurate but rarely needed in routine practice.
Where the exam tries to catch you
Estimation questions usually test the steady-state assumption, the drug that changes creatinine, or the muscle-mass trap.
- Trap: using eGFR to dose a renally cleared drug during acute kidney injury. Correct: equations assume a steady state, so while creatinine is rising the estimate overstates function and dosing needs caution.
- Trap: labelling a bodybuilder with creatinine 1.2 mg/dL as having CKD. Correct: high muscle mass raises creatinine, so the equation can underestimate GFR, and cystatin C is the better check.
- Trap: treating a trimethoprim-related creatinine rise as true nephrotoxicity. Correct: trimethoprim blocks tubular creatinine secretion, so the rise is artefactual and true GFR is unchanged.
Frequently asked questions
Why does creatinine clearance overestimate GFR when kidney function is low?
Creatinine clearance overestimates GFR at low levels of function because tubular secretion adds creatinine to the urine, and this contribution becomes proportionally larger as filtration falls. The clearance calculated from creatinine is therefore higher than the true GFR. This is one reason cystatin C and measured clearance are used in advanced disease.
Which equation is used to report eGFR in current practice?
The CKD-EPI equation is the standard eGFR used for reporting and staging chronic kidney disease. The 2021 version of CKD-EPI omits the race coefficient used in earlier versions. Cockcroft-Gault remains the usual method for drug dosing rather than for staging.
When is Cockcroft-Gault preferred over CKD-EPI?
Cockcroft-Gault is preferred for estimating creatinine clearance for drug dosing, because many drug labels use it. It uses age, weight, sex and serum creatinine, so the body weight chosen must be considered carefully in obesity. CKD-EPI is preferred for staging chronic kidney disease.
Why is cystatin C used when creatinine gives a misleading estimate?
Cystatin C is produced at a fairly constant rate by nucleated cells and is less dependent on muscle mass than creatinine. It is therefore useful in amputees, cachexia and extremes of body build. Steroids, thyroid disease and smoking can alter cystatin C, so its interpretation must be cautious.
