The decision to switch is guideline-automatic for symptomatic HFrEF on an ACE inhibitor. Order the last ramipril dose at 8 pm on Monday; first ARNI dose at 8 am Wednesday — 36 hours. Choose 49/51 mg twice daily because his ramipril dose was high. Check blood pressure, potassium and creatinine at 1-2 weeks, double to 97/103 mg twice daily if systolic pressure holds above 100 and potassium below 5.4. Warn him about symptomatic hypotension on standing in week one. If he had instead been ARB-experienced (losartan 100 mg), no washout is needed — switch directly at an equivalent-intensity starting dose, because ARB plus neprilysin inhibition was PARADIGM's own comparator arm's safety profile in practice.

The switch does not replace anything else: beta-blocker, mineralocorticoid antagonist and SGLT2 inhibitor layers continue, and ivabradine or devices follow their own triggers.

Where the switch goes wrong

The dangerous error is arithmetic on the washout: 36 hours means a day and a half, not "next dose"; and the error's mirror image is pharmacists or cross-cover teams starting an ACE inhibitor on top of an existing ARNI prescription, which is why the drug chart must show one agent discontinued in ink. Second, angioedema history: any prior angioedema — including hereditary or aspirin-related — contraindicates ARNI, since bradykinin accumulation is the shared final pathway; examiners pair this with the viva question on why ARNI angioedema risk is low in practice yet non-zero. Third, titration timidity: many Indian prescriptions fossilise at 49/51 mg when the target is 97/103 mg — under-dosing a proven therapy out of hypotension fear, when the correct move is check volume status, reassess timing, then titrate. A final exam pearl: neprilysin also degrades amyloid beta, and theoretical Alzheimer concerns have not translated into clinical trial signals — quotable as reasoned reassurance.
