Imagine a 32-year-old man on clozapine 450 mg/day for six months with plasma level 480 ng/mL, still hearing commanding hallucinations. Step one is hygiene, again: adherence by history and level concordance, thyroid and renal screen, cannabis use review, and a check that the target symptom is treatment-refractory psychosis rather than comorbid depression or post-psychotic demoralisation — augmenting the wrong target is the commonest real-world error.

Step two chooses one add-on with a stated rationale and duration. If the priority is persistent positive symptoms with distress, ECT augmentation is discussed openly — in Indian tertiary centres ECT access is good and cost low, making it more practical than in many Western services. If the priority is metabolic burden alongside partial response, aripiprazole 10 mg is added and clozapine may be trimmed slightly. Six to eight weeks later, one decision is made: continue what worked, or strip what did not. Two augmenting agents together (for example aripiprazole plus lamotrigine plus topiramate) is where prescribing drifts into polypharmacy, and the exam — like the clinic — wants it named as such.

Viva examiners press the sequence question: what must be true before clozapine itself is started (failure of two adequate antipsychotic trials of 4-6 weeks each at adequate doses), and what must be true before augmenting it (an adequate clozapine trial as above).
