Estimated GFR reported alongside every creatinine is now calculated race-free using the 2021 CKD-EPI creatinine equation, or the creatinine–cystatin C combined equation when greater accuracy changes a decision. Serum creatinine alone misleads at the extremes of muscle mass — the amputee, the frail elderly vegetarian, the bodybuilder — precisely where cystatin C rescues the estimate. Measured creatinine clearance overestimates true GFR by roughly 10–25 per cent because of tubular secretion, and every KDIGO staging decision pairs the GFR category (G1–G5) with the albuminuria category (A1–A3) before management is planned.

  • Equations: the 2021 CKD-EPI equations dropped the race coefficient; use creatinine-based for screening and the creatinine–cystatin C combination for borderline decisions such as toxic drug dosing or live kidney donor evaluation.
  • Cystatin C confounders: glucocorticoids, hyperthyroidism, obesity and inflammation raise cystatin C independently of GFR; it is freely filtered and catabolised in proximal tubules, so little appears in urine.
  • Creatinine blind range: GFR can fall from 120 to 60 mL/min/1.73 m² before creatinine leaves the laboratory reference interval — a "normal" creatinine never excludes CKD.
  • Tubular secretion blockers: cimetidine, trimethoprim and dronedarone raise creatinine without changing GFR — a classic steady-creatinine, rising-drug stem.
  • Creatinine clearance: a 24-hour collection overestimates GFR by 10–25 per cent (secretion adds to filtered load); cimetidine-blocked collection narrows the error.
  • Measured GFR: iothalamate, iohexol or EDTA plasma clearance remains the reference standard used in transplant donor work-ups and clinical trials.
  • De-indexing for drug dosing: multiply the reported eGFR by the patient's BSA/1.73 m², because drug labels were largely built on Cockcroft–Gault derived clearance.
  • Staging anchors: G1 ≥90, G2 60–89, G3a 45–59, G3b 30–44, G4 15–29, G5 <15 mL/min/1.73 m² — abnormality sustained beyond three months.

Working through a discrepant eGFR

A 58-year-old vegetarian woman weighing 46 kg has a creatinine of 1.1 mg/dL and a reported eGFR of 48 mL/min/1.73 m². Step one: confirm the value is chronic — retrieve older reports; a value of 0.7 a year ago makes this acute, not CKD. Step two: quantify albuminuria with a first-morning ACR, because G3a with A1 and G3a with A3 are different diseases in the heat map. Step three: send cystatin C; it returns an eGFR of 62 by the combined equation. The discordance points to low muscle mass generating creatinine-based underestimation — a familiar trap in Indian practice, where reference ranges imported from Western laboratories run high for a smaller-bodied population, and an unremarkable 1.0 mg/dL in a small-built woman can conceal G3 CKD. Step four: de-index — her BSA is 1.45 m², so her absolute GFR for drug dosing is roughly 62 × 1.45/1.73, about 52 mL/min. Step five: decide — SGLT2 inhibitor eligibility, NSAID counselling and a two-to-three-month recheck interval all follow from the paired GFR–albuminuria stage, not from the creatinine alone.

Where candidates slip

Three errors recur in the NEET-SS and INI-SS pool. First, equating creatinine clearance with GFR — the exam rewards knowing secretion accounts for the 10–25 per cent gap. Second, calling cystatin C "unaffected by anything except GFR" — steroids, hyperthyroidism and obesity break that claim. Third, quoting MDRD at high GFR, where it systematically underestimates; the 2021 CKD-EPI equations exist precisely to fix that. A favourite image-based viva question shows a kidney donor chart with an indexed eGFR of 85 in a 1.9 m² candidate — the correct move is de-indexing before accepting her.
